Senior Team Healthcare Assistant (Community)

About the Role

What Youll Do: Lead and coordinate care delivery in the community, ensuring safe, effective, and compassionate clinical

practice. Line manage, mentor, support, and supervise HCAs and nursing staff fostering a positive team culture focused

on learning and development. Develop and implement training and competency frameworks for the care team. Support the

integration and delivery of complex and specialist palliative care. Oversee assessments, care planning, and

implementation of individualised care in collaboration with patients, families, and the wider team. Manage complex

symptoms and end-of-life care with clinical skill and emotional sensitivity. Contribute to continuous service
