About the Role

An exciting opportunity has arisen for a Clinical Sister / Charge Nurse / Clinical Practitioner to join our Care

Navigation Centre. You must be a registered practitioner (nurse / paramedic) with evidence of post registration practice

at Band 5. Experience of multi-disciplinary / multi agency working is essential. You will be part of a team and have

responsibility for the triage and onward referral of patients who require support from the plethora of community

services available in Walsall. You will need to work flexibly to meet the demands of the service and be able to forge

strong links with colleagues from the community and acute services. You should have good IT skills and a pleasant
