About the Role

You will assist Dollar Park Dementia Service Team in providing services to older adults who have a diagnosis of dementia
to live in their own home for as long as possible with the best quality of life.
You will provide practical support and supervision within a small group setting and within a wider community setting.
You will be involved in the referral process, initial home assessments, maintaining accurate records, developing care
plans, reports on individuals and co ordinating groups activities. Rotas- allocating work to support workers,
