About the Role
Provide 1:1 personalisedsupport to patients referred for social prescribing across West Enfield GP practices - Work
collaboratively with GPs, nurses, mental health professionals, and the wider PCN workforce - Co-producepersonalisedcare
plans that focus on patient goals, strengths, and aspirations - Support patients to access community resources including
social groups, financial and housing support, employment services, wellbeing activities, and voluntary opportunities -
Strengthen relationships with localorganisationsandmaintainan up-to-date directory of community assets within Enfield -
Promote independence, confidence-building, and self-management for individuals facing social isolation, long-term
