About the Role

Job Summary We are seeking a compassionate and motivated Band 5 Social Prescribing Link Worker to join our

multidisciplinary team. This role is pivotal in supporting people with long-term health conditions, as well as children

and families, to improve their wellbeing through non-clinical, community-based support. As a Link Worker, you will work

in partnership with primary care professionals, local authorities, schools, and voluntary/community sector organisations

to empower individuals to take greater control of their health and wellbeing. You will address the wider social

determinants of health by connecting people with a range of services including community groups, parenting support,
