About the Role
about the Role As a Social Worker within our secure services, youll be a key driver of effective discharge planning,
reduced length of stay and improved patient progression. Youll work closely with the Discharge Liaison Nurse, MDTs and
external agencies to ensure every patient has a clear, coordinated pathway out of secure care. You will: Lead on social
care assessments and contribute to robust discharge planning Work proactively to identify, address and escalate barriers
to discharge Liaise with local authorities, community teams, MAPPA, safeguarding and partner agencies Ensure legal
frameworks MHA, MCA, Care Act are applied appropriately Support families and carers, ensuring their voices are heard in
