FULL TIME EXPERIENCED SOCIAL WORKERS

Hospital Discharge Team & Community Home First Hub Service.

An exciting opportunity is now available for the correct Social Work candidates to join a hybrid social care services

supporting patient in the acute setting and those patients placed to Discharge to Assess & Long-Term facilities.

This role is based over two facilities, one acute and one community which both support multidisciplinary working. As

part of this multidisciplinary approach, you will work hand in hand with therapy, nursing & reablement teams all focused
