36 hours per week, 52.14 weeks per year, Permanent

As part of the West Lothian Health and Social Care Partnership (HSCP), the Care Home Collaborative team is an integrated

social care team with social workers, community care coordinators, advanced nurse practitioners, band 6 nurses and a

bowel and bladder specialist.  The team completes initial and annual reviews of care and nursing home placements,

undertakes care management tasks and has statutory responsibility under Adult Support and Protection and Adults with
