About the Role

Duties and responsibilities Provide highly specialised advice concerning care for patients requiring anticoagulation

therapy in the inpatient and outpatient setting in line with Trust Policies, Nice Guidelines and NPSA18. Ensure that

documentation is completed to the highest standard and that this standard is maintained. Participate is the ongoing

development of the service and open to continual career development. To work within the anticoagulant service with

focus, innovation and best evidence-based practice providing patients with the necessary knowledge and support to be

able to manage their anticoagulant needs. To lead on providing education on anticoagulation and VTE Prevention training
