Specialist Children's Community Nurse
About the Role
Main responsibilities will include: Leading on supporting the childrens Community nursing team to meet the needs of
children within Special School Setting. Leading on strengthening relationships and collaborative working with partner
agencies to ensure children with complexity have their needs understood and safely met within their chosen setting
Leading on nurse led clinic initiatives working closely with Community paediatricians supporting school and community
clinics and reviews. Responsible for EHCP coordination within the CCN team. Ensuring quality person centred health
advice and appropriate identification of individual need identified Responsible lead clinician for the caseload of
