Specialist Clinical Rotational Pharmacist with EDC
About the Role
Provide a high-quality clinical pharmacy service across specialist and general ward areas in line with Trust policies,
current legislation, and departmental standards. Work closely with Lead Pharmacists, clinicians, nurses, and senior
managers to develop, implement, and evaluate pharmaceutical services and protocols to ensure safe, effective, and
cost-effective use of medicines. Assess pharmaceutical care needs, undertake medicines reconciliation and optimisation,
develop and implement care plans, review and adjust drug therapy, counsel patients, support discharge planning, and
provide medicines advice to healthcare staff. Attend ward rounds as required. Act as an independent prescriber in
