Specialist Dietitian - Adult Integrated Care

About the Role

To work as a member of the Adult Integrated Care Dietetic Team in community settings across Lambeth and Southwark To

provide dietetic care to adult patients who are undernourished requiring support with their oral nutritional intake.

This includes patients who are malnourished or at risk of malnutrition, those with pressure ulcers and patients

requiring nutritional support due to illness, long term conditions or social factor To communicate and work closely with

members of the health and social care teams, general practice teams and the voluntary sector to ensure malnourished

patients receive the appropriate care and support in the community To visit patients whom are housebound at home, and
