The applicant will need to work autonomously and be able to manage their own time and group of patients. The applicant

will be based on Capener ward (Eastern) but will be working mainly in the Emergency department and the Acute Frailty

Same Day Emergency Care Unit. The role involves assessing patient for Frailty on admission and implementing a plan of

care, alongside the Frailty Multi disciplinary team. The aim is to ensure patient centred care, focusing on discharging

to a safe environment as soon as possible. Working Pattern: Hours to be discussed at interview. The team work mainly
