Specialist Nurse Looked After Children and Care Leavers
About the Role
The post holder will support the Named Nurse for Looked After Children in delivering a high-quality, effective, and
compassionate health service for Looked After Children, Care Leavers, and other vulnerable groups. They will contribute
to improving health outcomes by ensuring statutory requirements are met and that children and young people receive
timely, holistic, and child-centered care. The role includes undertaking statutory health assessments in a variety of
settings, including home, clinic, and community environments, and developing, implementing, and reviewing individualised
health care plans. The post holder will manage their own caseload, ensuring effective prioritisation and coordination of
