About the Role

responsible for own practice and accountable for all aspects of treatment delivered to the client to ensure a high
standard of clinical care. To work as an autonomous lone practitioner, case managing clients in the community and
coordinating their care. To carry out timely assessment and rehabilitation of community clients to prevent hospital
admission or re-admission and facilitate early discharge home from hospital. Management and Leadership To deputise in
some tasks in the absence of the Senior Specialist Nurse if required. Communication To work in a client centred manner
