The aim of the Intermediate Care Lambeth is to provide community based care and rehabilitation to prevent unnecessary

hospital admission, facilitate early hospital discharges and promote independence reducing reliance on long term social

care services.

The successful applicant will work alongside Physiotherapists, Occupational Therapists, Social Workers, Social Care
