About the Role
The successful candidate will work as part of a dynamic and flexible multidisciplinary team, delivering high-quality,
responsive care to patients across the Virtual Ward. The role also includes in-reach responsibilities within the acute
hospital and Accident & Emergency departments to identify patients suitable for step-up or step-down care within the
Virtual Ward.
There is an expectation of flexibility to work across the Virtual Ward, Urgent Community Response, and acute hospital
settings, supporting seamless transitions of care and ensuring patients receive the right care, in the right place, at
