Our diverse, talented and caring team include frailty consultants, consultant advanced clinicalpractitioners (ACP), GPs,

consultants-in-training, ACPs and trainee ACPs from a broad professional background. Together we support a wide range of

activities. The Urgent Community Services are co-located and work collaboratively.

Research tells us that people prefer to receive their care closer to home and that most people wish to stay at home for

as long as possible.

The seven-day service provides expert frailty adviceand input throughout West Kent. Receiving referrals from any
