About the Role

Job Description Summary The core purpose of the role is to facilitate timely discharges, improve patient flow, and

ensure a safe transition for patients leaving the hospital. Key Responsibilities Patient Assessment and Care Planning:

Conducting comprehensive nursing assessments, developing, implementing, and evaluating evidence-based programmes of

care. Discharge Coordination: Coordinating and facilitating patient transfers from wards/departments to the discharge

lounge and their eventual destination (home, care facility, etc.). This includes liaising with social services,

therapists, transport services, and pharmacy. Medication Management: Administering medications as needed and ensuring
