The newly formed Out of Hours Urgent Community Response (OOH UCR) team combines the patients traditionally seen by both

District Nursing and our Intensive Home Support Service. This is an exciting time to join the service as we come

together to enhance the level and breadth of care and treatment we are able to deliver to our patients in their own

home, preventing unnecessary hospital admissions and meeting the national guidance around Urgent Community Responses.

The post holder will contribute a high level of skilled nursing care to patients within the community setting. They will
