About the Role

The successful candidate will contribute to the assessment of each service user and their individuals needs and

formulate appropriate socially inclusive, collaborative recovery programmes, incorporating robust risk management and

positive behaviour support plans to enable safe therapeutic care delivery in the community.

A joint professional approach to person centred care is essential and a willingness to ensure the service user is at the

centre of the support they need to live a full and rewarding life focusing on promoting independence, choice,

opportunities and community inclusion.
