About the Role

To work in close collaboration with other professionals in an Medium Secure setting, assessing patients needs, devising

Person Centered care plans and evaluating outcomes that promote patient empowerment through the process. Alongside the

wider Multi disciplinary Team you will be expected to participate in providing high quality, individualised care, which

addresses the needs and wants of adolescents and adults with Learning Disabilities and/orpsychiatric disorders or

challenging behaviours.
