Staff Nurse - SMART Team
About the Role
The SMART Service is nurse led, therefore the ability to work autonomously as well as effectively within a team is
essential. The work will include visiting patient in the community therefore it is essential that you have a full UK
driving license and a car. You will triage, assess, plan and implement the care of patients who are discharged home
under the care of the SMART service in the community, review and give care to SMART patients attending SDEC/ Hot clinic
for review, administer IV treatments and conduct virtual visits using our remote patient monitoring pathways. The Staff
Nurse must also be able to liaise with all members of the multidisciplinary team to coordinate safe effective and timely
