Discharge Team as a full time band 3 Therapy assistant, supporting and rehabilitating our stroke patients in the

community.

You can make a real difference to patients' lives and help them build a future following a stroke.

Your role will involve working alongside highly specialised Stroke Occupational Therapists, Physiotherapists and Speech

and Language Therapists in the community
