Supportive and Palliative Care Coordinator
About the Role
Assist in the coordination and booking of appointments and assessments as required including SPCT ward follow ups and
new Enhanced Supportive Care referrals to the team. Record outcomes of symptom assessment patient questionnaires on
behalf of the SPCT OP team and ensure they are correctly uploaded onto the EPR Assist in the collection and recording of
outpatient and inpatient activity data required for defined SPCT Key Performance Indicators (KPIs). Able to work
autonomously. Refer to Palliative Care CNS when necessary. Monitor and record patients put onto Patient Initiated Follow
Up (PIFU) list following divisional protocol Support and contribute to audit processes, governance, research, clinical
