About the Role

This post is for the Team Manager of the Care Home Team. The Team is an Integrated Team responsible for supporting older

people's care homes and the service users who reside there. The Team consists of Social Workers, Registered Mental

Health Nurses, Registered General Nurses and an Occupational Therapist. Working in a hybrid style, based both at home
