About the Role

Telehealth is the means by which technologies and related services concerned with health and wellbeing are accessed by

people or provided for them irrespective of their location.

ELFT’s telehealth services are offered by its Telehealth Team. The services provide care for people with COPD, diabetes,

heart failure and hypertension and are based on a step up/step down model. The Telehealth service is provided to local

service users and integrated with the wider community services and other stake holders i.e. primary care and secondary

care services aiming to deliver the right quality care at the right place, at the right time including digital.
