Therapy Liaison - Neighbourhood Rehabilitation Service

About the Role

JOB SUMMARY KEY RESULT AREAS: The postholder will: Work within community services and support the multi-disciplinary

team (MDT) for Neighbourhood Rehabilitation Service. Contribute in ensuring that safe care is provided to patients

within a community setting. Enhance the admission and discharge processes for patients and their families/carers.

Support the clinical team with the admission to and discharge from Neighbourhood Rehabilitation Service; ensuring a

smooth transition throughout the appropriate elements of community Intermediate Care services. Work in close liaison

with the Intermediate Care patients key clinician / MDT in ensuring an efficient admission process and effective
