About the Role

Working closely within the VTE/Anticoagulation Nurse team with the Haematology consultants & anticoagulation Pharmacist,

the CNS provides an anticoagulation dosing and advice service, for patients on Vitamin K Antagonists and direct oral

anticoagulants. Our client group includes a variety of patients with pro-thrombotic conditions requiring safe &

effective management of their anticoagulation. This care is delivered across both hospital sites. To assist with the in-

patient management of the complex patients providing specialist knowledge and ensuring transition of anticoagulation
