About the Role
SERVICE DELIVERY Undertake comprehensive health care needs assessment of patients, reassessing as appropriate.
Facilitate patients to take an active role and support in advice/treatment provided. To provide a resource of advice and
expertise to community patients, their families, carers and other health care professionals as appropriate Implement and
monitor systems and processes to ensure that patient needs are comprehensively assessed, appropriately planned,
effectively implemented and evaluated according to changing health care needs and varying levels of complexity Implement
