Are you an established nurse or AHP looking to develop your skill set and be part of a large transformation project?

This is a great time to be part of The Transfer of Care Hub service, sharing your clinical expertise and having your

ideas heard, whilst reviewing the discharge process for both the acute and community settings.

As the Transfer of Care Hub Clinician, you will provide a consistent presence within the acute settings, Poole and

Bournemouth Hospital on behalf of Dorset HealthCare for the Transfer of Care hub. You will be responsible for clinically

triaging referrals sent to single point of access (SPA) within Dorset Healthcare Trust and liaising with all teams
