About the Role

To provide community nursing support to young adults who have a learning disability during the transition from children

to adult services. The post holder will work across a variety of community settings to deliver nursing assessment,

intervention and review to individuals who may have complex presentations. Participate in research and development

activity and contribute to the overall development of learning disability services. Provide supervisory support to

junior nurses and unqualified members of the team.
