Twilight Community Staff Nurse

About the Role

Together with your Team, you will deliver high quality nursing care in the community setting out of hours, providing

whatever short-term input is needed within the scope of practice under guidance if required from the shift co-ordinator.

In liaison with our day team colleagues, we aim to support & maintain our patients in their own homes. The post holder,

as part of the team will provide the care needed whatever that may be. It might be end of life care for the patient to

ensure they achieve their preferred place of death, supporting them, their family & friends to meet that goal. Urgent

visits for newly acquired trauma wounds, The post holder will manage the clinical needs of the identified patients as
