About the Role
Join the Urgent Community Response (UCR) Team as a Nurse, where you'll play a vital role in preventing unnecessary hospital admissions. You'll assess and prescribe care pathways for patients with long-term conditions, ensuring quality of life and independence. Collaborate with senior colleagues and various healthcare professionals to deliver integrated care and support early hospital discharges.
Responsibilities
- Assess, plan, implement, and evaluate patient care.
- Develop individualized care pathways for each patient.
- Communicate complex information to patients and carers.
- Liaise with GPs and other healthcare professionals.
- Participate in multi-disciplinary meetings.
Requirements
- Registered Nurse qualification.
- Experience in community nursing or urgent care.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Knowledge of infection control policies.
