About the Role
JOB OVERVIEW
City Health Care Partnership CIC are looking for highly motivated, enthusiastic individuals to join a developing team of
practitioners in the Urgent Treatment Centre. The post holder will be responsible for the assessment, diagnosis,
treatment of urgent, and primary care patients (via telephone, face to face or a digital platform) with presentations
including undiagnosed undifferentiated illness and minor injuries, exacerbation of long term conditions, and acute
presentations. You will work collaboratively with partner organisations to ensure that care is seamless and timely to
