Urgent Care Response Support Worker

About the Role

We are seeking to recruit an Urgent Response Support Worker.

The successful candidate will be a key member of a multi-disciplinary team providing support to adults who are returning

to their own home after a stay in hospital or from reablement unit. These individuals require support to regain their

independence during times when there is an alteration in their physical or psychological well-being.

This role provides a high standard of support within a reabling ‘ethos’ which promotes independence and improved quality
