About the Role

Responsibilities
- Prevent unnecessary hospital admissions through community care.
- Assess and prescribe care pathways for patients in crisis.
- Act as a patient advocate, facilitating choice and empowerment.
- Provide evidence-based clinical interventions.
- Support early hospital discharges and seamless service integration.
- Triage referrals for the Urgent Community Response Team.
Requirements
- Registered nurse with relevant community nursing experience.
- Strong communication and holistic assessment skills.
- Full driving license and access to a vehicle.
- Ability to work weekends and flexible hours.
- Experience in palliative and end-of-life care preferred.
