About the Role

Join the North Urgent Community Response Team as a registered nurse, providing essential care to patients in their homes. Responsibilities

• Prevent unnecessary hospital admissions through community care.
• Assess and prescribe care pathways for patients in crisis.
• Act as a patient advocate, facilitating choice and empowerment.
• Provide evidence-based clinical interventions.
• Support early hospital discharges and seamless service integration.
• Triage referrals for the Urgent Community Response Team.

Requirements

• Registered nurse with relevant community nursing experience.
• Strong communication and holistic assessment skills.
• Full driving license and access to a vehicle.
• Ability to work weekends and flexible hours.
• Experience in palliative and end-of-life care preferred.
