About the Role
Community Independence Service Urgent Community Response and adjunct pathways for admission avoidance.
CNWLandWest Londonare working together in a Tri-Borough Capacity and this recruitment is a joint campaign to mobilise
new service pathways and the development of a hospital at home adjunct to our current UCR services. We work together
across the Inner London Borough and our teams work closely together to provide intermediate care services to our
populations.
