About the Role
An exciting opportunity has arisen to work as a Band 5 Community Nurse for the Virtual Ward within the Central Cheshire
Integrated Care Partnership Care Community Teams.
Working in partnership with Care Community Teams, Primary Care Services and Mid Cheshire NHS Foundation Trust to ensure
the smooth and timely enrolment of patients deemed suitable to receive safe care and treatment in their own home. The
post holder will be part of a team responsible for the care and support of all Virtual Ward patients.
