About the Role

We are excited to offer an opportunity to join the dynamic and forward-thinking Frailty Virtual Ward team in Newham

University Hospital. This innovative service is at the forefront of delivering integrated, patient-centred care to some

of our most vulnerable residents, supporting older adults with frailty to receive safe and effective treatment in the

comfort of their own homes.

As part of the Frailty Virtual Ward, you will be working within a multidisciplinary team that includes GPs,
