About the Role
Role Specific Responsibilities Developing and delivering a health screening tool which will identify CYPs physical
health needs Triaging of cases to prioritise need and plan of care. Signposting to appropriate services, as and when
required. Acting as an advocate for the CYP, where additional support is required Developing a database of all CYP
engaged with YJS and at risk adolescent services to enable delivery of the health screening tool, capturing of
outcomes/outputs, and inform commissioning and needs analysis Joint working with YJS colleagues including the provision
of an overview of health needs (with CYPs consent) Joint working with Children Looked After (CLA) service, including
